The Kansas High School Athletic Association (KSHSAA) requires all student-athletes to complete a physical packet before they are allowed to participate in any athletic competition. A complete physical packet includes the following elements:
- Completed PPE Forms
- Completed Doctor’s Physical Examination
- Signed KSHSAA Concussion and Head Injury Release Form
- Signed SHHS Concussion Baseline Testing (SWAY) Consent Form
- Signed Student Transportation Form
- Completed Emergency Medical Information / Insurance Verification Form
To be considered valid for the 2025-2026 school year, physicals must be dated after May 1, 2025. Physicals obtained for the previous school year are not valid. High school students should not use the middle school physical forms. The high school forms are slightly different.
No athlete will be allowed to practice until ALL forms have been turned into the office and marked complete. If you have any questions, please call the high school at 913-592-7299.
