The Kansas High School Athletic Association (KSHSAA) requires all student-athletes to complete a physical packet and return it to the SHHS office. A complete physical packet includes the following elements:

• Completed PPE Form
  
  • (Parent signatures are now required on pages 4 and 5.
• Completed doctor's page (Newest Revision 3/24)
  
  • (Now requires one doctor's signature on page 4)
• Signed 2024-25 Concussion Form
• Completed Emergency Contact Form 
  
  • (Must have complete insurance information including policy number)
• Signed Concussion Vital Signs Form
• Signed Transportation Form
  
  • (Minimum of two parent/guardian signatures)
  • If your student is driving to school - Sign all three lines)

SHHS student-athletes should not use the middle school forms. The high school forms are slightly different.

Please note that physicals obtained for the previous school year are not valid for the 2024-2025 school year. To be valid for the upcoming school year, physicals must be dated after May 1, 2024. Spring Hill High requires a copy of ALL forms, SIGNED, and on file in the office prior to the start of practice. A student missing paperwork or signatures cannot practice until everything is complete.
