The Kansas High School Athletic Association (KSHSAA) requires all student-athletes to complete a physical packet and return it to the SHMS office. A complete physical packet will include the following forms:
- Completed PPE Form (Must be revision 4/2023)
- Completed Dr.’s page (Completed/dated after 05/01/2023)
- Signed Parent/Guardian page
- Signed 2023-24 Concussion page
- Completed Emergency Contact Page (Must have completed insurance information)
Please note that physicals obtained for the previous school year are not valid for the 2023-24 school year. To be valid for the upcoming school year, physicals must be dated after May 1, 2023. Spring Hill Middle requires a copy of ALL forms, SIGNED, and on file in the office prior to the start of practice.
A student missing paperwork or signatures cannot practice until everything is complete.

Activities
Students are strongly encouraged to participate in extracurricular and co-curricular activities programs at Spring Hill Middle. The wide range of activities that is available to students helps them explore interest areas and develop talents. Students also enhance their citizenship skills and practice teamwork, which prepares them for life and their continued education.
All activities at Spring Hill Middle are designed to benefit all of the students involved. Our goal is not only to teach the student participants about the activity but also how it relates to life and how their experiences may help them become more successful in life. By emphasizing basic skills, teamwork, cooperation, effort, and citizenship, sponsors and staff believe that these activities will instill pride, confidence, and build positive self esteem in each student participant.
At our school, students have the opportunity to participate in a long list of activities. These range from academic competitions and leadership opportunities to activities related to the fine arts, like band, strings, choir, theater and more. Community service also is strongly emphasized at Spring Hill Middle, including giving back to the local community and sharing globally.

Concussion Awareness & Prevention
The safety of all students is and will continue to be our top priority. Head injury and concussion education, prevention, and management efforts are part of our commitment to meet this expectation. All SHMS coaches for fall, winter, and spring sports have completed the online course, "Concussion in Sports - What You Need to Know" which is provided free by the National Federation of High School Sports.
Parents and Guardians are invited to also take this online course:
